It has happened in the PTB as well. I get a safe unhook, i'm not being effected by monstrous shrine, and I still can't get off the hook. Sometimes it works, sometimes it doesn't. I have no idea what causes it. It even shows the increased Luck, but I can still try to get off 3 times, and not getting off. It just doesn't work.

Yeah, I've had multiple safe unhooks with deliverance tier 1 and it has yet to actually work. Maybe deliverance 2 and 3 work?
I have Deliverance 2 in the live version. And that is bugged for me. I did have Deliverance 3 in the PTB, but I still got it from time to time. So I think it goes across all of the tiers.
I heard rumours about luck offerings affecting Deliverance...
Might be the same with luck based perks.
I just now registered to report on this. I reported on it back during the PTB and was surprised it's still there. I've tried it at every level, and now, on multiple characters. Never successfully unhooked with it.
This perk is not working as intended. Even when the luck symbol appears, it doesn't let me unhook myself. I think it was improperly coded.
Happened in PTB for me too.
