Self-Unhooked then Insta-Died on Next Hook

I was on the verge of going into phase two of my hook progress and tried for a successfuly self-unhook. After this happened I was hooked again and died instantly.

What I think happened was I waited long enough for the bar to go below halfway and since I unhooked myself it registered me as being unhooked in phase two.

    It happened to me too. First hook on the basement, unhooked myself at second try and when he hooked me again, insta died
